Non-Medical Support Disclaimer

Sundry Care, LLC (“Sundry Care,” “Sundry,” “we,” “us,” or “our”) provides non-medical home care, companionship, homemaking, personal care assistance, and certain coordination and support services, depending on the service selected and the location in which services are provided.

Our Services Are Non-Medical

Unless expressly provided through an appropriately licensed healthcare professional and specifically identified in writing, Sundry Care services are non-medical in nature.

Our caregivers and coordinators do not diagnose medical conditions, prescribe treatment, provide medical advice, administer medications, perform skilled nursing procedures, provide physical or occupational therapy, or replace the services of a physician, nurse, pharmacist, therapist, emergency medical professional, or other licensed healthcare provider.

Caregivers may provide medication reminders when authorized and appropriate, but Sundry Care caregivers do not administer medications as part of our non-medical home care services.

Care Coordination

When Sundry Care assists with appointment scheduling, provider communication, prescription refill coordination, transportation arrangements, appointment accompaniment, family communication, or similar coordination services, we are helping organize and facilitate services.

Such assistance does not constitute medical advice, medical treatment, case management by a licensed clinician, or a guarantee regarding the quality or outcome of services provided by an independent healthcare provider or other third party.

Clients remain responsible for medical decisions made in consultation with their licensed healthcare providers.

Third-Party Services

At a client's request, Sundry Care may help identify, schedule, or coordinate third-party services such as transportation, home maintenance, salon services, wellness services, or other community resources.

Unless otherwise expressly stated in writing, these third-party providers are independent from Sundry Care. Sundry Care does not control their professional judgment, availability, pricing, performance, or service outcomes.

Emergencies

Sundry Care is not an emergency response service and should not be used in place of 911 or other emergency services.

If you believe that you or another person is experiencing a medical, safety, or life-threatening emergency, contact 911 or the appropriate emergency service immediately.

Changes in Condition

Clients and their authorized representatives should promptly notify Sundry Care of significant changes in a client's health, mobility, living environment, medication routine, safety needs, or other circumstances that may affect the safe delivery of services.

Sundry Care may recommend a reassessment, modification of services, or referral to an appropriate healthcare professional when needs appear to exceed the scope of non-medical services we provide.

Individual Service Agreements Control

This website provides general information about Sundry Care's services. Actual services, schedules, rates, responsibilities, limitations, and other terms are governed by the applicable Service Agreement, Care Plan or Care Approach Summary, Statement of Work, membership agreement, and other documents executed between Sundry Care and the client.

If information on this website conflicts with an executed client agreement, the executed agreement controls to the extent permitted by applicable law.
